Dietitian Travel/Contract Position

Location: Cottonwood, Arizona
Contract Length: 13 weeks

Job Details

• Specialty: Dietary Services (Clinical Dietitian)
• Shift: Day shift, 5 days a week (Monday-Friday), 8 hours per day (5x8s)
• Some Saturdays may be required depending on patient census
• Average patient visits per day: 12

• Guaranteed 40 hours per week; facility may call off up to 6 times during the 13-week assignment

Required Qualifications

• Valid Registered Dietitian Nutritionist (RDN) license in Arizona
• Minimum 1 year of experience as a Registered Dietitian Nutritionist (RDN), with experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association

Required Documentation & Compliance

• Valid Driver’s License or State ID (passport alone not accepted)
• Background checks including 7-year history, SSN trace, criminal, sex offender, EPLS/GSA/SAM, and HHS/OIG within 30 days of start
• Education verification required (diploma or transcripts accepted if verification pending)
• Employment application covering last 3 years with documented gaps over 30 days
• Two professional references (one from most recent employer within 1 year)
• Physical exam within 1 year confirming ability to perform essential job functions
• Drug screening: 10-panel test within 30 days of start, updated annually
• Immunizations and health requirements:

  • Negative TB test or chest x-ray within 1 year, updated annually
  • Tdap vaccine preferred within last 10 years or declination
  • Measles, Mumps, Rubella (MMR) immunity by titer or 2 vaccines
  • Varicella immunity by titer or 2 vaccines
  • Hepatitis B immunity by titer, vaccine series, or declination
  • Seasonal flu vaccine or signed declination form (mask required if declined)
  • COVID vaccine series or signed declination form
• Fit mask (N95) within 1 year, updated annually; facility may provide onsite fit testing if needed
• Completion and signing of facility-specific job description, confidentiality agreement, substance abuse policy, professional attire policy, and security question form prior to start

• First-time travel healthcare workers are accepted
• Unit manager approval required for time off requests
• No prior travel experience required for this position
