Job Summary

This role provides physical therapy services in a home health and hospice setting, with swing bed and outpatient opportunity. The therapist will assess patients, develop treatment plans, deliver rehabilitative interventions, coordinate with an interdisciplinary team, and educate patients and caregivers.

Key Responsibilities

• Perform comprehensive physical therapy assessments and develop individualized treatment plans.
• Supervise and coordinate care with PTAs.
• Provide interventions focused on mobility, strength, pain reduction, and functional independence.
• Collaborate on patient-centered care and discharge planning.
• Educate patients and caregivers on safe mobility, equipment use, and home exercise programs.

Required Qualifications

• Valid Physical Therapist license in North Dakota
• Driver's License
