Physical Therapist - Outpatient (Travel/Contract)
Location: Alamosa, Colorado
Facility Type: Acute Care Hospital (49 staffed beds)
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Contract Length: 13 weeks
Job Summary
- Outpatient physical therapy position with some inpatient floating required (approximately one half-day inpatient on Fridays, floating between two nearby facilities about 15 miles apart).
- Weekend coverage required on a rotating basis approximately once every two months.
- Guaranteed 40 hours per week.
- Block scheduling is not accommodated; position is Monday through Friday.
- Locals and first-time travelers are accepted.
Required Qualifications
- Valid Physical Therapist license in Colorado.
- Minimum 2 years of experience as a Physical Therapist, with experience within the last year.
Compliance and Documentation Requirements
- Background check within 90 days of start, including SS trace, county searches (7 years), nationwide sex offender check, and criminal history review. Felonies and certain misdemeanors subject to hiring manager discretion.
- Physical exam within 1 year of start date (annual updates not required).
- Fit mask test within 12 months of start (must include make, model, and size). On-site testing available if needed.
- Negative TB test (T-Spot or QuantiFERON Gold) within 45 days of start; PPDs not accepted. For past positive TB, a negative chest x-ray within 1 year is required.
- Immunizations/vaccinations required:
- Tdap within last 10 years
- Measles, Mumps, Rubella: 2 vaccines or positive titer; boosters required if titers negative or equivocal
- Hepatitis B: positive titer or completed vaccine series; booster and retesting if titers negative/equivocal
- Varicella: titer, 2 vaccines, or history of disease
- Drug screening: Negative 10-panel drug test with chain of custody within 30 days of start (instant drug screens accepted).
- Seasonal flu vaccination required during flu season (Oct 1 - Mar 31); declinations not accepted.
- Colorado CAPS (Child Abuse and Neglect) check required; must be current with no gaps over 30 days between assignments.
- Photo for badge required.
- Competency exams required, including medication/pharmacology test; must meet or exceed passing score (minimum 80% if no Angoff score provided).
- CO CAPS Release Authorization form must be completed within 48 hours of signing and CAPS clearance obtained prior to start.
Additional Information
- No time off (RTO) allowed within the first two weeks of assignment; total RTO cannot exceed 10 days and no full calendar weeks off allowed.
- Mileage reimbursement provided for travel between facilities.
- Free parking available in any unrestricted space.
- Charting systems used: OBix, McKesson, Pyxis, and SBAR.
- Scrub color is flexible (any color allowed).
This position offers a stable, full-time outpatient physical therapy role with some inpatient duties, suitable for therapists with at least 2 years of experience and appropriate licensure, who are comfortable floating between nearby facilities and meeting comprehensive compliance requirements.
