Registered Nurse (RN) - Home Health (Days)

Job Description

• Provide nursing care through assessment, planning, implementation, and evaluation following professional nursing standards.
• Supervise CNAs and coordinate care with other healthcare workers as assigned.
• Conduct comprehensive initial and ongoing assessments covering physical, psychosocial, environmental, and community resources across the lifespan.
• Initiate, re-evaluate, and modify Plan of Care as needed.
• Establish discharge plans collaboratively with patients, physicians, and caregivers.
• Complete OASIS data and documentation according to regulatory guidelines.
• Assess educational needs of patients, families, and caregivers and implement appropriate teaching strategies.
• Promote a clean, safe environment using error prevention habits.
• Maintain patient privacy and confidentiality in compliance with standards of conduct.
• Perform Telemonitoring/Virtual visits in addition to home visits.

Required Qualifications

• Valid Registered Nurse (RN) license in Illinois.
• Minimum of 1 year of Home Health experience, with recent experience within the last year preferred.
• Basic Life Support (BLS) certification.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Valid driver’s license.
