Job Description

Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders.
Serve as Case Manager when acting as the lead discipline on a case.
Deliver home health and hospice care focused on holistic, community-centered support.
Support a range of services including health promotion, long-term care, and maternal-and-child health.

Required Qualifications

Valid RN license in Vermont
Minimum of 1 year of Home Health experience, including experience within the last year
BLS certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
