- Guaranteed 40 hours per week

Service Area
- Berks County
- May require coverage in other territories within the facility’s service area, up to about a 60-mile radius from the facility address
Required Qualifications
- Valid Physical Therapist license in Pennsylvania
- Experience in a Home Health setting
- Homecare Homebase documentation experience
- Experience with OASIS
Assignment Details
- Average 4–6 visits per day, depending on visit type
- Productivity expectation: 28–32 points per week
- Overtime must be pre-approved in writing by the administrator
- 2-week cancellation notice
Notes
- The client may consider candidates without Homecare Homebase experience at a reduced rate for the first 2 weeks if the manager is willing to train
