Hospice RN Travel Contract

Location

• Washington, Pennsylvania

Contract Length

• 13 weeks

Schedule

• Day shift, 5x8 hours
• 8:00 AM to 5:00 PM

Job Summary

• Provides and coordinates home nursing care based on physician orders, agency policies, and the nursing process.
• Supervises and evaluates home health aides and licensed practical nurses/licensed vocational nurses.
• Work includes home visits and periodic office meetings.
• Typical caseload is 5–8 patients per day.

Required Qualifications

• Valid RN license in Pennsylvania
• Associate of Science in Nursing / Associate Degree in Nursing
• Hospice experience within the last year
• 1 year of hospice experience
• Experience in a Home Health setting
• One completed travel assignment
• Experience with Homecare Homebase
• Experience with OASIS
• Current BLS/CPR certification issued by the American Heart Association or American Red Cross
• Driver's License
• Reliable transportation
• Agency-required liability insurance

Work Expectations

• Must be willing to travel up to 75 miles from the home clinic
• All on-call assignment
• Weekend requirements vary by location and profession

Charting / Documentation

• Must be proficient with HomeCare HomeBase within the last 6 months
• Hospice RN candidates who have not used OASIS within the last 6 months must complete OASIS E course

• Local candidates are accepted
