Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in a Long Term Care setting

Role Summary

• Screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care
• Develops and adjusts plans of care using standardized assessments and evidence-based practice
• Provides treatment interventions such as manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
• Documents patient progress, treatment minutes, discharge summaries, and other required clinical information in the electronic documentation system
• Communicates with the supervisor and care team about patient progress, barriers, and treatment plans
• Participates in required meetings, trainings, in-services, and conferences
• May supervise physical therapist assistants, rehabilitation aides, and/or students
