Required Qualifications

Valid Physical Therapist license in Hawaii
Physical Therapist (2 years of experience)
Physical Therapist experience within the last year
Bachelor's degree
Experience in a(n) Home Health setting
Driver's License
Experience with Epic

Preferred Qualifications

Master’s degree
Travel experience

Job Responsibilities

Plan and conduct treatment programs to restore and improve physical, social, and mental functions
Complete evaluations and address areas needing attention
Develop individualized treatment and discharge plans
Provide therapy interventions using appropriate protocols and techniques
Complete accurate clinical documentation within established deadlines
Supervise assigned assistants
Serve as co-leader in an interdisciplinary approach to patient groups
