Job Overview

Responsibilities

• Provide home health physical therapy services
• Manage start-of-care and follow-up visits
• Travel between patient homes within the assigned territory

Required Qualifications

• Valid Physical Therapist license in New York
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Experience working with Adult patients
• Experience in a Home Health setting
• Driver's License

Work Expectations

• Average 5–6 patients per day
• Productivity expectation: 30 points per week
• Must be willing to travel up to 75 miles from the home clinic
• Mileage is reimbursed at the IRS standard rate
• Local candidates are accepted

Schedule Notes

• Weekend rotation may vary by location and profession
• No on-call requirement listed
• Some assignments may use a Baylor shift schedule of 4x10s, Friday–Monday

Additional Notes

• First-timers are not accepted
• PointCare is the EMR/system used
• Physical exam, TB screening, immunization documentation, drug screen, references, and work history verification are part of the onboarding process
