Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in a(n) Long Term Care setting

Key Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care
• Develop and adjust plans of care using standardized assessments and evidence-based practice
• Perform rehabilitation interventions such as manual techniques, ambulation, therapeutic exercises, modalities, supportive/assistive devices, and wound care
• Document treatment, progress, outcomes, discharge summaries, and required regulatory information in the electronic documentation system
• Communicate with the care team, patients, families, and caregivers about progress and treatment plans
• Participate in required meetings, trainings, in-services, and conferences
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students
