Registered Nurse - Home Health

Role Overview

Contract home health RN position in Mesa County, Colorado
Full-time, Monday-Friday day schedule with on-call rotation
In-person role providing skilled nursing care in patients’ homes

Required Qualifications

Valid RN license in Colorado
Home Health experience within the last year
Home Health (1 year of experience)
BLS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License

Preferred

Home health experience

Key Responsibilities

Perform comprehensive patient assessments
Develop and update individualized plans of care
Provide skilled nursing care and appropriate interventions
Monitor patient progress and report changes to the care team
Educate patients and families on medications, treatments, and care plans
Coordinate care with LPNs, therapists, caregivers, and providers
Complete timely and accurate documentation
Participate in on-call rotation
