Inpatient Pharmacist

Job Details

  • Location: Cambridge, Massachusetts
  • Employment type: Long-term assignment
  • Contract length: 19 weeks
  • Shift: 8:00 AM - 4:30 PM

Required Qualifications

  • Valid Pharmacist license in Massachusetts
  • Pharmacist (1 year of experience)
  • Pharmacist experience within the last year

Onboarding Requirements

  • Primary source verification of professional license and/or certification within 30 days of start date and updated upon expiration.
  • Criminal background check within 45 days prior to start date and updated after a break in service.
  • Massachusetts CORI check within 30 days prior to start date and updated after a break in service.
  • SSN trace.
  • National sex offender check within 30 days of start date and updated annually.
  • Exclusion checks within 30 days of start date and annually.
  • Education verification of the highest level achieved.
  • MGB medication exam.
  • Orientation summary.
  • Confirmation of assignment/POI form.
  • Beeline attestation.

Health Requirements

  • MMR immunity or vaccination documentation.
  • TB screening documentation within 3 months of start date, with updated documentation after a break in service.
  • Health statement signed by an MD, DO, NP, or PA within 365 days of start date, with updated documentation after a break in service.

Optional

  • Positive PPD with negative chest X-ray documentation, if applicable.
  • Name change documentation, if applicable.
  • TB symptom survey, if applicable.
