Description

Physical Therapist, Behavioral Health

Location

  • Staten Island, New York

Schedule

  • Day shift
  • 2x10 hours
  • 20 hours per week
  • Part-time hours: Monday through Friday, 10:00 AM to 2:00 PM

Contract Length

  • 26 weeks

Required Qualifications

  • Valid Physical Therapist license in New York
  • BLS - Basic Life Support issued by American Heart Association or American Red Cross

Background and Screening Requirements

  • Statewide Central Register Database Check Form
  • Criminal background check from the hiring agency
  • Fingerprinting required
  • Background check covering the last 7 years of locations lived, worked, or attended school, with searches updated annually

Health and Compliance Requirements

  • Physical exam within 1 year of start
  • Negative TB test or chest X-ray within 12 months before start
  • TB questionnaire annually
  • Drug test within 1 year of start
  • Flu vaccine or declination for the current flu season
  • COVID-19 booster preferred, not required
  • Measles, mumps, rubella, hepatitis B, and varicella documentation or titers
  • If not immune to measles or rubella, MMR booster required

Preferred Experience

  • Behavioral health, rehabilitation, acute care, or skilled nursing experience

Key Responsibilities

  • Perform physical therapy evaluations and develop individualized plans of care
  • Provide therapeutic exercise, gait, balance, mobility, transfer, and strengthening interventions
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients, families, and staff on mobility, fall prevention, exercises, and assistive devices
  • Collaborate with physicians, nursing, behavioral health, and other interdisciplinary team members
  • Maintain timely and accurate clinical documentation
  • Participate in care coordination and discharge planning
  • Follow HIPAA, infection prevention, safety, and facility policies
