Job Details

- Weekend rotation every 4–6 weeks

- Coverage area: Van Buren, Calhoun, and Kalamazoo counties
Required Qualifications
- Valid Physical Therapist license in Michigan
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association
- Experience in a Home Health setting
- Driver's License
Responsibilities
- Evaluate patients and provide physical therapy treatment to improve function, strength, pain, and independence
- Assess functional abilities using standardized testing
- Develop treatment plans and patient education
- Provide direct patient care, including individual and group therapy activities
- Document evaluations, treatment plans, treatments, and progress in the electronic health record
- Maintain accurate, legible, and complete records
- May cross-train and provide coverage in other rehabilitation departments as needed
