Required Qualifications
- Valid Physical Therapist license in Massachusetts
- 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
- Experience in a Home Health setting
- 1 year of Case Management experience
Schedule
- Weekend rotations occur once every 6 weeks
Role Summary
- Provide physical therapy services in patients’ homes
- Collaborate with patients, families, physicians, and other allied health professionals to coordinate care
- Complete electronic documentation and interdisciplinary communications according to agency policies and standards
Onboarding / Compliance
- Resume including the past 7 years of work experience and city/state for work and education
- 2 references within the past 4 years, including at least 1 supervisor or above in the skillset
- Skills checklist completed within 1 year of submission
- Primary source verification of license and/or certification
- Background checks and registry checks required for onboarding
- N95 fit test and respirator medical clearance, if required for the skill
- Immunization and health documentation required for onboarding
