Job Overview
- Location: Seaside, Oregon
- Employment type: Travel/Contract
- Start date: ASAP
- Shift: Day shift, 8 hours
- Schedule note: Facility cannot accommodate 4x10s or 3x12s
- Setting: Home health
- Contract length: 13 weeks
Required Qualifications
- Valid Physical Therapist license in Oregon
- 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association or Heart and Stroke Foundation
- Experience in a Home Health setting
Schedule and Work Expectations
- Weekend work not required
- On-call not required
- No floating between like units
- Block scheduling not available
- May visit up to 6 patients per day
- Standard productivity: 30 points
- Traveler must have their own transportation
- Occasional travel radius of roughly 60 miles per day
Location and Candidate Preferences
- Local candidates accepted within a 50–75 mile radius
Compliance and Health Requirements
- Primary series COVID vaccination plus one booster
- Physical exam within 1 year of start
- Fit mask required
- Tdap within 10 years
- Negative TB test or chest X-ray
- Measles, mumps, rubella, varicella, and hepatitis B documentation required
- Drug screen required
- Flu vaccination or declination required during flu season
- Respirator clearance required
Additional Notes
- Free parking
- Mileage reimbursed at IRS standard rate
- Business casual attire; scrubs allowed but not required
- Charting uses EPIC Remote Client for Home Health, Rover, and Oasis
