PT Home Health
Job Overview
- Travel/contract physical therapy role in home health
- Day shift, 5x8 schedule
- 40 hours per week
- Home visits with periodic office meetings
- Start of care and follow-up visits
- Average caseload: 5–6 patients per day
Required Qualifications
- Valid Physical Therapist license in New York
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Driver's License
- Experience with Homecare Homebase
Assignment Details
- Contract length: 13 weeks
- Local candidates accepted
- Must be willing to travel up to 75 miles from the home clinic
- Mileage is reimbursed at the IRS standard rate
- Travel time is compensated from patient home to patient home
- The commute to the first and last patient is not billable
- Weekend rotation varies by location and profession
- On-call requirement varies by job/location
- Some positions may use a Baylor shift schedule of 4x10s Friday–Monday
- Most schedules are standard 5x8s Monday–Friday
Compliance / Pre-Start Items
- Background check required
- Education verification required
- Two references required
- Work history must verify at least the 2 most recent employers, or the last 7 years if only one employer exists in that period
- Physical exam required within 12 months of start and updated annually
- Negative TB test or chest X-ray required within 12 months of start and updated annually
- Drug screen required
- COVID vaccination or declination required
- Flu vaccination documentation or declination required
- Hepatitis B documentation required
- Varicella documentation required
- Rubeola, mumps, and rubella documentation required
- Confidentiality and privacy acknowledgements required before start
- Home health-specific compliance documents required before start
- Personal or auto insurance documentation required for home health placements

- Weekly productivity expectation: 30 units
- Territory includes Westchester and the northern area
