Overview
- Contract role in Lewistown, Montana
- 13-week assignment
- 36 hours per week, day shift
Required Qualifications
- Valid Physical Therapist license in Montana
- Physical Therapist experience within the last year
- 2 years of Physical Therapist experience
- BLS
- Driver's License
Role Summary
- Receive and review physician therapy orders for Home Health and Hospice patients
- Complete patient evaluations and create written treatment plans with short-term and long-term goals
- Provide physical therapy interventions such as therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
- Assess home safety and recommend equipment, assistive devices, or environmental modifications
- Monitor patient progress and update treatment plans as needed
- Educate patients, caregivers, and family members on treatment plans, home exercise programs, safety measures, and expected outcomes
- Maintain clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
- Collaborate with physicians, nurses, therapists, and other healthcare professionals
- Travel to patient homes and other care settings to provide therapy services
