Description

Pride Health is actively hiring an Occupational Therapist (OT) to join our client's Home Health Team.

Location: Marion, IN 46952
Contract Length: 13 Weeks (Option to Extend)
Patient Care Setting: Home Health
Schedule: Monday-Friday, 8:00 AM - 4:30 PM EST
Hours: 40 Hours/Week (36 Hours Guaranteed)
Productivity Expectation: 30 Points Per Week
On-Call Requirement: None
Weekend Requirement: None
Mileage Reimbursement: Yes, reimbursed between patient visits
Coverage Area: Wabash, Huntington, Grant, Blackford, Madison, and Delaware Counties
Dress Code: Navy Blue Scrubs

Position Overview:

  • We are seeking a dedicated and experienced Home Health Occupational Therapist (OT) to provide skilled therapy services to patients in their homes throughout Marion and surrounding counties.
  • The Occupational Therapist will assess, evaluate, and treat patients with functional limitations, helping them regain independence and improve their quality of life while collaborating closely with the interdisciplinary care team.

Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations and reassessments in the home setting.
  • Conduct OASIS evaluations, recertifications, discharge assessments, and routine therapy visits.
  • Assess patients' functional status, activities of daily living (ADLs), fine motor coordination, cognitive abilities, safety needs, and adaptive equipment requirements.
  • Develop individualized treatment plans in collaboration with physicians and interdisciplinary team members.
  • Implement evidence-based occupational therapy interventions to maximize patient independence and functional performance.
  • Perform home safety assessments and recommend adaptive equipment, environmental modifications, and therapeutic strategies.
  • Observe, document, and communicate patient responses to treatment, progress toward goals, and changes in condition to physicians and nursing staff.
  • Coordinate care with nurses, therapists, physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes.
  • Complete accurate and timely clinical documentation in compliance with agency, CMS, and state regulations.
  • Meet established productivity standards while maintaining high-quality patient care.

Required Qualifications:

  • Current Occupational Therapist (OT) License in the State of Indiana.
  • Current CPR Certification.
  • Minimum 1 year of recent Home Health experience.
  • Proficiency with HomeCare HomeBase (HCHB)/PointCare.
  • Strong experience completing OASIS assessments.
  • Valid driver's license, current automobile insurance, and reliable transportation.
  • Ability to work independently and effectively manage a field-based caseload.
  • Ability to provide care to patients of all backgrounds regardless of age, race, creed, disability, or socioeconomic status.
