Description

Occupational Therapist Pediatrics

Overview

  • Travel/Contract role in Willmar, Minnesota
  • 13-week assignment
  • Day shift: 4x10 hours, 7:30 AM–6:00 PM
  • Mostly pediatric hospital outpatient with some adult inpatient coverage as needed
  • Working with both adult and pediatric patients with a wide variety of diagnoses
  • Outpatient clinic is attached to the hospital via a tunnel and is across the street

Required Qualifications

  • Valid Pediatric Occupational Therapist license in Minnesota
  • Pediatric Occupational Therapist (1 year of experience)
  • Pediatric Occupational Therapist experience within the last year

Required Credentials and Compliance

  • Occupational Therapist license, if applicable
  • BLS certification, if applicable
  • Physical exam within 1 year of start
  • Fit mask required
  • Tdap vaccination required
  • Negative TB documentation required
  • Rubeola (Measles) immunity documentation required
  • Mumps immunity documentation required
  • Rubella immunity documentation required
  • Varicella immunity documentation required
  • Hepatitis B vaccination, positive titer, or declination required
  • Flu vaccination documentation or declination required during flu season
  • Criminal background check required within 1 year of start
  • GSA/SAM excluded provider check required
  • OIG excluded provider check required
  • Third-party education verification required
  • Photo for badge required
  • Confidentiality agreement required
  • Skills checklist required
  • Two references from recent employer(s) within 2 years of start required
