PT Home Health
Overview
- Travel/Contract role in Merrimack, New Hampshire
- Day shift: 5x8 hours, 8:00–16:30
- 40 hours per week
- 13-week contract
- No weekends
- First-time travelers are accepted
- Local candidates within 50 miles are considered local
Required Qualifications
- Valid Physical Therapist license in New Hampshire
- Physical Therapist (2 years of experience)
- Physical Therapist experience within the last year
- Experience with Epic
- Required skills: lifting and transfer, patient transport to/from treatment, scheduling appointments, telephone calls
Preferred
- Order supplies
- ER, ICU, Med/Surg support unit experience
Additional Requirements
- SSA release required
- Background check required within 30 days of start
- Social Security Verification required within 30 days of start
- EPLS/GSA/SAM must be run 30 days prior to start
- HHS/OIG must be run 30 days prior to start
- Education verification required
- Transcript and diploma may be accepted if education verification is pending or cannot be completed
- State license copy required for states that are not paperless
- State license verification required
- Employment application required
- Skills checklist required
- Two current professional references within the last 3 years, including one supervisor reference
- Physical exam within 1 year prior to start
- Annual fit mask test required for all clinical candidates
- Tdap required
- Negative TB test or chest x-ray required
- TB questionnaire required
- Drug screen required within 14 days prior to start
- Flu vaccination or approved exemption required during flu season
- RespSafety clearance required
- Photo for badge required
- Personal or auto insurance required
- BAAS State Registry Results required for New Hampshire placements
