Description
Operating Room
Overview
- Travel/Contract role in Kingman, Arizona
- 13-week assignment
- Day shift: 4x10 hours, 6:45–17:15
- 40 hours per week
- Call and late call required, averaging an estimated 5–6 shifts per month depending on staffing
- Locals and first-time travelers are accepted at the same rate
- Minimal to no holiday RTO
Required Qualifications
- Valid RN license in Arizona
- OR - Operating Room (2 years of experience)
- OR - Operating Room experience within the last year
- ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- PALS - Pediatric Advanced Life Support issued by American Heart Association or American Red Cross
- Associate of Science in Nursing / Associate Degree in Nursing
- Government-issued photo ID
- 7-year background check within 30 days of start, updated annually
- EPLS/GSA/SAM check, required annually
- HHS/OIG check, required annually
- Transcript or diploma
- Education verification through NSCH
- State license verification
- Medication test/pharmacology competency exam, as applicable by profession
- Competency exam, as applicable by profession/specialty
- Facility-specific job description
- Physical exam within 1 year of start
- Fit test within 1 year of start, updated annually
- Negative TB or chest X-ray within 1 year of start
- Annual TB questionnaire
- Measles, mumps, and rubella immunity documentation or MMR documentation for candidates born in 1957 or later
- Hepatitis B titer, declination, or vaccine series
- Varicella titer, 2 vaccines at least 4 weeks apart, or history of chicken pox
- 9-panel drug screen within 30 days of start
- Flu vaccine or declination during flu season
- 2 references within 3 years of start
- Skills checklist, updated annually
Preferred
- NRP, as required

- Charting system: Meditech
- A clear, passport-style badge photo with a solid background is required
- Facility-specific forms and training are required, including confidentiality, behavioral expectations, right to know, HIPAA/information security, and agency candidate/attestation forms
