Description

Physical Therapist

Overview

  • Specialty: Rehabilitation Therapy
  • Setting: Home Health
  • Location: Missoula, Montana
  • Travel: Requires travel to patients’ homes within the service area
  • Schedule: Typically Monday–Friday, 8:00am–5:00pm, with occasional evening/weekend coverage
  • Contract length: 12 weeks

Required Qualifications

  • Bachelor's degree in Physical Therapy
  • Valid Physical Therapist license in Montana
  • Physical Therapist experience within the last year
  • 1 year of Physical Therapist experience
  • BLS
  • Driver's License
  • Experience in supervisory roles
  • Experience with wound care

Preferred Qualifications

  • Two years of previous PT experience

Role Responsibilities

  • Evaluate and provide rehabilitation intervention in the home setting
  • Manage and supervise patient care
  • Provide patient and family education
  • Coordinate care with other disciplines and obtain orders for additional services as appropriate
  • Provide direct patient care based on the care plan and physician orders
  • Complete OASIS scoring and collaborate with the Quality team on needed adjustments
  • Perform interventions allowed under the physical therapy practice act, including wound care
  • Oversee services provided by the Physical Therapist Assistant and complete supervisory visits within the required time frame
  • Take supervision of CNA when RN is no longer involved in the care plan
  • Maintain documentation, time logs, and mileage records
  • Participate in quality improvement, multidisciplinary conferences, and agency education opportunities
