Description
Registered Nurse Home Care
Overview
- Contract role in Battle Creek, Michigan
- Shift: Monday–Friday, 8:00 am–4:30 pm
- Provides direct nursing care to participants in a PACE home care setting
- Works independently and may manage complex clinical situations
- Includes clinic and home-based care, home visits, on-call needs, and travel between centers as needed
Required Qualifications
- Valid RN license in Michigan
- Associate of Science in Nursing / Associate Degree in Nursing
- Home Health experience within the last year
- Experience working with geriatric patients
- Current BLS certification issued by the American Heart Association
- Current valid Michigan driver’s license
- Current valid vehicle insurance
- Reliable transportation
- Must be 18 years of age or older
- Must be fully vaccinated, or willing to be fully vaccinated, against Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B
Preferred Qualifications
- Bachelor of Science in Nursing
- Minimum of one year home care experience
- Minimum of one year wound care experience
Skills and Experience
- Proficiency with Microsoft Word and Outlook
- Strong written and verbal communication skills
- Ability to maintain confidentiality
- Ability to make independent decisions
- Ability to plan, organize, develop, implement, and interpret goals, objectives, policies, and procedures
- Ability to use time and resources efficiently and effectively
- Ability to provide exceptional customer service
Work Environment and Responsibilities
- Triages emergent situations
- Monitors acute and chronic medical conditions in coordination with Primary Care Providers
- Administers treatments, procedures, medications, immunizations, and other care as prescribed
- Performs wound care, assessments, lab draws, and other nursing tasks
- Works beyond normal hours, including weekends and holidays as needed
- May be exposed to infectious waste, diseases, hazardous chemicals, TB, AIDS, and Hepatitis B virus
- Subject to callback during emergency conditions
- Must be able to drive in all types of weather
- Must be able to lift up to 20 pounds
