Job Summary

Seeking highly motivated and compassionate Home Health Physical Therapists to join a dynamic clinical team. The role focuses on helping patients return to and maintain their optimum level of physical functioning through full life care, addressing all stages and dimensions of well-being.

Job Responsibilities

• Provide physical therapy services to patients as prescribed by a physician, following agency policies and federal/state regulations.
• Perform initial and ongoing assessments, including OASIS assessments, to evaluate patient functioning and ability to perform activities of daily living.
• Develop and revise plans of care in consultation with physicians and care team members to help patients achieve realistic mobility goals.
• Report patient reactions to treatment and any changes in condition to the physician and agency representatives.

Required Skills & Experience

• 1 to 2 years of experience.
• Minimum of 1 year of experience in home health care preferred.

Education & Licensure

• Doctor of Physical Therapy (DPT) from a CAPTE-accredited program required.
• Licensure as a Physical Therapist in the state of Florida required.
• Basic Life Support (BLS) certification from the American Red Cross or American Heart Association required.
