Registered Nurse - Hospice (Travel)

Location: Basking Ridge, New Jersey
Employment Type: Clinical - Nursing - Travel
Contract Length: 13 weeks

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals. Develop and revise plans of care for patients and families.
• Ensure appropriate utilization of services and maintain visit productivity standards. Demonstrate knowledge of insurance reimbursement guidelines.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods for patients and families; evaluate and document patient education tools used.
• Manage case management responsibilities including developing, implementing, and evaluating evidence-based plans of care. Prioritize care needs and communicate progress with the interdisciplinary team (IDT). Attend IDT meetings every 2 weeks.
• Maintain a safe environment by following infection control measures and reporting infections. Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in monthly staff meetings, performance improvement programs, utilization review meetings, and other committees as requested. Provide orientation and mentorship to new employees and cross-train to learn and teach new skills.
• Assist with agency growth by supporting orientation and accepting additional assignments as needed.

Qualifications & Skills

• 2-3 years of experience as an RN in a home hospice environment.
• Current New Jersey RN license.
• Valid New Jersey driver’s license.
• BLS certification required.
• Experience using an Electronic Medical Record (EMR).
• Excellent critical thinking and communication skills.
