Hospice RN - Pennsylvania (Irwin, PA)
Job Summary
- Provide direct patient care services to hospice patients and supportive care to their families.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
- Case manage all assigned patient clinical records.
- Perform initial and ongoing patient assessments.
- Develop and implement the Hospice Plan of Care in collaboration with physicians and the interdisciplinary team.
- Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report changes in patient condition to attending and hospice physicians.
- Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
- Practice infection control measures and standard precautions.
- Monitor staff scheduling and participate in quality assessment and performance improvement programs.
- May participate in agency on-call schedule.
- Maintain knowledge of pain and symptom management in the dying process.
- Conduct routine supervisory visits with team members at least monthly.
- Review and follow up on patient clinical records to ensure quality and compliance with policies and regulations.
- Maintain close contact with patients’ families to provide information, support, and continuity of care.
- Act as primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, hospice philosophy, and care of the terminally ill.
Contract Length
- Approximately 13 weeks (89 days rounded to 13 weeks).
Required Skills & Experience
- Minimum of 1 year RN experience in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered).
- Knowledge of terminally ill patients and hospice care needs.
- Understanding of roles of all hospice disciplines and excellent patient assessment skills.
- Familiarity with community resources, strong oral and written communication, documentation, and program management skills.
- Knowledge of general nursing practice, managed care principles, and regulatory guidelines (Medicare, Medicaid, ACHC).
- Ability to operate nursing assessment tools and common office equipment.
- Must have and maintain transportation for work.
