Job Opportunity: Inpatient Care Manager (Travel Nursing)

Location: Newton, New Jersey

Job Summary

The RN Inpatient Care Manager coordinates care and services for selected patient populations to ensure safe and timely transitions to the next appropriate level of care. Responsibilities include clinical assessment, planning, implementation, coordination, monitoring, and evaluation of patient care options. The role involves collaboration with the healthcare team to address clinical, psychosocial, and financial needs, promoting effective utilization of healthcare resources and optimal patient outcomes.


Key Responsibilities

  • Develop comprehensive case management plans by reviewing clinical findings and collaborating with physicians.
  • Identify high-risk patients requiring psychosocial intervention and refer to social services.
  • Conduct patient assessments to prevent readmissions and implement strategies to reduce future readmissions.
  • Develop and document discharge/transition plans in collaboration with the interdisciplinary team, ensuring continuous updates to meet patient needs.
  • Monitor and escalate financial management issues affecting post-acute care plans.
  • Facilitate patient progression by monitoring length of stay, participating in care rounds, and addressing discharge barriers.
  • Coordinate transitions of care by communicating with patients, families, physicians, and care team members to ensure seamless care across settings.
  • Maintain knowledge of federal and state regulations, including CMS rules and The Joint Commission standards.
  • Provide education and resources to patients, families, and healthcare teams regarding care progression and safe transitions.
  • Identify and address potential risk management or quality issues in collaboration with the healthcare team.

Required Skills & Experience

  • Excellent communication, negotiation, and conflict resolution skills.
  • Proficiency in data management and computer skills.
  • Knowledge of relevant state and federal regulatory standards related to case management.
  • Expertise in rapid cycle change or clinical performance improvement.
  • Prior clinical experience in care and management of hospitalized patients.

Preferred Skills & Experience

  • Experience in acute care case management.
  • Case management training from a professional Case Management organization.
