Description
Job Opportunity: Social Worker II
Position Overview
Provide social services to patients and their families under general supervision. Responsibilities include discharge planning, psychosocial assessments, crisis intervention, counseling, and acting as a liaison between patients, families, hospital staff, and community agencies.
Key Responsibilities
- Provide social services for an assigned medical service.
- Conduct discharge planning evaluations and psychosocial assessments.
- Counsel patients and families regarding diseases, treatments, social/financial situations, emotional stress, and other issues.
- Conduct high social risk case findings and provide crisis counseling related to child abuse, neglect, physical abuse, etc.
- Provide crisis intervention services.
- Assist with discharge planning and referrals to community services.
- Support patients and families with post-hospital placement or transfer to post-acute care.
- Assist with difficult adjustments to illness, treatment, or changes in functional status.
- Use specialized communication skills, including with children.
- Advocate for patients on issues such as informed decision making, end-of-life care, and ethical dilemmas.
- May supervise students or newly hired employees.
- May prepare statistics related to referrals and extended stays.
Experience & Skills
- Hospital experience preferred.
- Knowledge of social and psychological casework principles and practices.
- Familiarity with social and welfare resources in the community and hospital.
- Understanding of current social and economic conditions, trends, and problems.
- Strong analytical skills to interpret situations and determine appropriate actions.
- Ability to explain technical information clearly to patients, families, and staff.
- Good interpersonal skills.
Contract Length
- Approximately 13 weeks (89 days rounded to nearest week).
