Job Opportunity: Registered Nurse (RN) - Care Centers (Per Diem)

Location: Summerfield, Florida
Specialty: RN - Care Centers - Per Diem
Employment Type: Clinical Nursing - Hospice
Contract Length: 8 weeks (53 days ≈ 8 weeks)


Job Summary

The Care Center RN provides preventive and supportive care to terminally ill patients and their families under physician orders. Responsibilities include patient/family assessment, care planning, teaching, and coordination of care. The RN oversees LPNs and aides in delivering patient care within the General Inpatient (GIP) environment and collaborates closely with the care center manager, patients, families, and interdisciplinary team members.


Key Responsibilities

  • Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments.
  • Develop, implement, and update individualized Plans of Care in collaboration with patients, families, interdisciplinary teams, and physicians.
  • Provide care coordination and collaboration among all involved parties.
  • Administer, monitor, and document medications via all routes using age-specific care practices.
  • Identify patient and caregiver learning needs and provide appropriate teaching.
  • Promote a culture of safety by adhering to policies, infection control standards, and best practices.
  • Document all care processes timely and accurately in the electronic medical record (EMR).
  • Follow palliative care and end-of-life care principles with compassion.
  • Direct and oversee care provided by LPNs and Home Health Aides.
  • Participate in 24/7 patient care coverage as part of the team.

Required Qualifications

  • Experience: Minimum of 1 year of current active nursing experience.
  • Education: Associate’s degree (A.A.) or equivalent from a two-year college or technical school.
  • Licensure & Certifications:
    • Valid Florida Registered Nurse license.
    • Valid Florida State Driver’s License.
    • Valid BLS certification (American Red Cross or American Heart Association).
  • Physical Requirements:
    • Ability to lift, pull, push up to 50 lbs and exert force up to 100 lbs.
    • Physical stamina for long shifts and physically demanding tasks.
    • Ability to ambulate and stand for long periods.
    • Ability to bend, squat, kneel, reach, and perform coordinated hand/finger movements.
    • Visual and auditory acuity to perform patient assessments and monitor alarms.
  • Transportation: Reliable transportation required for timely task completion; proof of valid automobile insurance required.
  • May be required to float between multiple Care Centers.

Preferred Qualifications

  • Experience caring for terminally ill patients or those with life-threatening illnesses.
  • At least 1 year of experience in an acute or post-acute care setting (or equivalent education/experience).
  • Ability to type 25 words per minute is highly preferred.
