Hospice RN - North Carolina

Location: Fayetteville, North Carolina
Employment Type: Staffing - Supplier Sourced
Contract Length: 12 weeks (81 days ≈ 12 weeks)

Job Summary

• Provide direct patient care services to hospice patients and supportive care to their families.
• Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
• Case manage all assigned patient clinical records.
• Perform initial and ongoing patient assessments.
• Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team members.
• Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
• Report changes in patient condition to attending and hospice physicians.
• Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
• Practice infection control and standard precautions.
• Monitor staff scheduling and participate in quality assessment and performance improvement programs.
• May participate in agency on-call schedule.
• Maintain knowledge of pain and symptom management in the dying process.
• Conduct supervisory visits with team members at least monthly.
• Maintain close contact with patients’ families to provide information, support, and continuity of care.
• Act as primary liaison between physician, patient/family, and hospice team.
• Educate patients, families, and caregivers on medication administration, dressing changes, oxygen use, hospice philosophy, and terminal care.
• Supervise aides and perform home visits at least every two weeks to assess care and services.

Required Skills & Experience

• Minimum 1 year RN experience in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered).
• Knowledge of care for terminally ill patients and their families, with understanding of hospice needs.
• Strong patient assessment skills and knowledge of roles within hospice disciplines.
• Familiarity with community resources, documentation, program management, and regulatory guidelines (Medicare, Medicaid, ACHC).
• Proficient in nursing assessment tools and basic office technology (computers, fax, copier, cell phone).
• Must have and maintain transportation for work.
