Location: Summit, New Jersey
Employment Type: Travel Contract

Job Summary
The RN Inpatient Care Manager coordinates care and services for selected patient populations to ensure safe and timely transitions to the next appropriate level of care. Responsibilities include clinical assessment, planning, implementation, coordination, monitoring, and evaluation of patient care options. The role involves collaboration with the healthcare team to address clinical, psychosocial, and financial needs, promoting effective utilization of healthcare resources and optimal patient outcomes.
Essential Duties
- Develop comprehensive case management plans by reviewing clinical findings and collaborating with physicians.
- Identify high-risk patients requiring psychosocial intervention and refer to social services.
- Conduct patient assessments to prevent readmissions and implement strategies to reduce future readmissions.
- Develop and document discharge/transition plans in collaboration with the interdisciplinary team, ensuring plans adapt to changing patient needs.
- Monitor and escalate financial management issues affecting post-acute care plans.
- Facilitate patient progression by monitoring length of stay and participating in care rounds to meet clinical and discharge targets.
- Coordinate transitions of care by communicating with patients, families, physicians, and care team members to ensure seamless care across settings.
- Maintain knowledge of federal and state regulatory requirements, including CMS and Medicare rules.
- Provide education and resources to patients, families, and healthcare teams regarding care progression and safe transitions.
- Identify and address potential risk management or quality issues in collaboration with the healthcare team.
Required Qualifications
- Valid Registered Nurse (RN) license in New Jersey
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum of 1 year of case management experience, including recent experience within the last year
- Experience in an inpatient setting
- Knowledge of relevant state and federal regulatory standards related to case management
- Expertise in rapid cycle change or clinical performance improvement
- Acute hospital case management experience
