Description
Hospice RN
Location: North Charleston, South Carolina
Employment Type: Clinical Nursing
Job Summary
The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team, case managing assigned patient clinical records, and ensuring the highest quality of care in accordance with the Plan of Care. The RN will perform initial and ongoing patient assessments, develop and implement hospice care plans, and collaborate closely with the interdisciplinary team to provide comprehensive physical, psychosocial, educational, emotional, and spiritual support, including bereavement services.
Additional duties include:
- Reporting changes in patient condition to physicians
- Maintaining accurate and updated medical records
- Practicing infection control and standard precautions
- Participating in quality assessment and performance improvement programs
- Possibly participating in on-call schedules
- Supervising and supporting team members
- Educating patients, families, and caregivers on care needs and hospice philosophy
Contract Length
Approximately 14 weeks
Required Qualifications
Experience
- Minimum of 1 year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered in some cases)
- Knowledge of care for terminally ill patients and their families, including hospice needs
- Understanding of roles of all disciplines in hospice services
- Strong patient assessment skills
- Familiarity with community resources, documentation, and program management
- Knowledge of general nursing practice, managed care principles, and regulatory guidelines (e.g., Medicare, Medicaid)
Education
- Registered Nurse (RN)
Certifications & Licensure
- Current nursing license valid in South Carolina
- BLS (American Heart Association) certification
- Valid driver’s license and current automobile insurance
Additional Requirements
- Ability to operate nursing assessment tools and common office equipment
- Must have reliable transportation for work-related travel
