Description

Hospice RN

Location: Fort Wayne, Indiana
Employment Type: Clinical Nursing


Job Summary

The Registered Nurse (RN) will provide direct patient care to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team, case managing assigned patient clinical records, and ensuring the highest quality of hospice care in accordance with the Plan of Care. Key duties include:

  • Performing initial and ongoing patient assessments
  • Developing and implementing the Hospice Plan of Care with the interdisciplinary team
  • Providing physical, psychosocial, educational, emotional, and spiritual support, including bereavement support
  • Reporting changes in patient condition to physicians
  • Maintaining accurate and updated medical records
  • Practicing infection control and standard precautions
  • Monitoring staff scheduling and participating in quality improvement programs
  • Potential participation in on-call schedules
  • Supervising and supporting team members through routine visits
  • Educating patients, families, and caregivers on care needs and hospice philosophy

Contract Length

  • 13 weeks

Required Qualifications

Experience

  • Minimum of 1 year of Hospice Palliative Care RN experience within the last year
  • Experience in Acute Care, Long Term Care, Home Health, and Oncology settings
  • Knowledge of general nursing practice, including thorough knowledge of managed care principles, regulatory guidelines (Medicare, Medicaid, ACHC), and human resource management principles

Education

  • Associate of Science in Nursing / Associate Degree in Nursing

Certifications & Licensure

  • Valid Registered Nurse (RN) license in Indiana
  • BLS certification issued by the American Heart Association

Additional Information

  • Must have reliable transportation for work-related travel
  • Position is safety-sensitive and requires compliance with annual training
  • Strong communication skills, both oral and written, are essential
