Location: Meadville, Pennsylvania

Job Summary

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team, case managing assigned patient clinical records, and ensuring the highest quality of care in accordance with the Plan of Care. The RN will perform initial and ongoing patient assessments, develop and implement hospice care plans, and collaborate closely with the interdisciplinary team to address physical, psychosocial, educational, emotional, and spiritual needs, including bereavement support.

Additional duties include:

• Reporting changes in patient condition to physicians
• Maintaining accurate and updated medical records
• Practicing infection control and standard precautions
• Monitoring staff scheduling and participating in quality improvement programs
• Potential participation in on-call schedules
• Supervisory visits and ongoing review of patient records to ensure compliance and quality
• Providing education to patients, families, and caregivers regarding care needs and hospice philosophy
• Acting as the primary liaison between physicians, patients/families, and the hospice team

Contract Length

Approximately 13 weeks

Required Qualifications

Experience

• Minimum of 1 year of Hospice Palliative Care RN experience within the last year
• Experience in acute care, oncology, long-term care, and home health settings
• Knowledge of documentation and program management

Education

• Associate of Science in Nursing / Associate Degree in Nursing

Certifications & Licensure

• Valid Registered Nurse (RN) license in Pennsylvania
• BLS certification issued by the American Heart Association

Additional Information

• Participation in annual compliance training required
• Must maintain a high level of knowledge related to pain and symptom management in end-of-life care
