Location: Monroe, Michigan

Contract Length: Approximately 13 weeks

Job Summary

• Provide direct patient care to hospice patients and supportive care to their families.
• Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
• Case manage all assigned patient clinical records, ensuring documentation aligns with the Plan of Care.
• Perform initial and ongoing patient assessments.
• Develop and implement the Hospice Plan of Care in collaboration with physicians and the interdisciplinary team.
• Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
• Report changes in patient condition to attending and hospice physicians.
• Maintain accurate and updated medical records, including interdisciplinary notes and assessments.
• Practice infection control and standard precautions.
• Monitor staff scheduling and participate in quality assessment and performance improvement programs.
• May participate in on-call schedule.
• Maintain knowledge of pain and symptom management in end-of-life care.
• Conduct monthly supervisory visits with team members.
• Ensure compliance with policies, procedures, and regulatory requirements through ongoing review of patient records.
• Maintain close contact with patients’ families to provide information, support, and continuity of care.
• Act as primary liaison between physician, patient/family, and hospice team.
• Educate patients, families, and caregivers on medication administration, dressing changes, oxygen use, hospice philosophy, and terminal care.

Schedule

• Monday to Friday, 8:00 AM to 5:00 PM
• Call one weeknight and one weekend day or night every other week
• Approximately 6 patients per day with regular visits
• Travel within a 40-mile radius
• Experience with Hospice Care Home-Based (HCHB) software preferred

Required Qualifications

• Valid RN license in Michigan
• Minimum 1 year of hospice experience, including experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• BLS certification issued by the American Heart Association
• Experience in Long Term Care settings
• Experience in Home Health settings
