Registered Nurse - Hospice

Location: Morristown, New Jersey
Employment Type: Travel Clinical Nursing
Contract Length: 13 weeks

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop, implement, and evaluate individualized plans of care for patients and families based on evidence-based knowledge.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement and hospice regulations.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education tools used.
• Prioritize patient care needs and communicate progress with the interdisciplinary team (IDT). Participate in IDT meetings every two weeks.
• Perform home health aide (HHA) supervisions per standards.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in performance improvement programs, utilization review meetings, and provide orientation and mentorship to new employees.
• Cross-train to learn new skills and assist with agency growth initiatives as assigned.

Schedule

• Shift: PM (11:00 AM – 7:00 PM)
• Hours per day: 7
• Hours per week: 37.5
• Work every other weekend

Required Qualifications

• Valid RN license in New Jersey
• Hospice Palliative Care RN experience with a minimum of 2 years, including experience within the last year
• BLS certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in a Home Health setting
• General Inpatient (GIP) experience within a hospital setting

Preferred Qualifications

• 3+ years of RN experience overall

Additional Information

• Associate's (ASN) degree or Diploma with RN License in New Jersey required
• Participation in monthly staff meetings and performance improvement activities expected
