Hospice RN - North Carolina

Job Summary
- Provide direct patient care to hospice patients and supportive care to their families.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Case manage all assigned patient clinical records.
- Perform initial and ongoing patient assessments.
- Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team.
- Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report changes in patient condition to attending and hospice physicians.
- Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
- Practice infection control and standard precautions.
- Monitor staff scheduling and participate in quality assessment and performance improvement programs.
- May participate in on-call schedule.
- Maintain knowledge of pain and symptom management in end-of-life care.
- Conduct supervisory visits with team members at least monthly.
- Review and follow up on patient clinical records to ensure quality and compliance.
- Maintain close contact with patient families to provide support and continuity of care.
- Act as primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, and hospice philosophy.
- Supervise aides and perform home visits at least every two weeks to assess care.
- Prepare written assignments for aides.
Required Qualifications
- Valid RN license in North Carolina
- Associate of Science in Nursing / Associate Degree in Nursing
- Minimum of 1 year hospice experience within the last year
- Experience in Long Term Care setting
- Experience in Home Health setting
- Acute Care experience
- Oncology experience
- BLS certification issued by the American Heart Association
- Experience with Homecare Homebase
Additional Information
- This is a safety-sensitive position.
- Completion of required annual compliance training is mandatory.
- Attendance at staff meetings is required.
