Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Nursing (Clinical - Nursing - Travel)
Contract Length: 16 weeks

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop, implement, and evaluate individualized plans of care for patients and families based on evidence-based knowledge.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement and hospice regulations.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education tools used.
• Assume case management responsibilities including prioritizing care needs, scheduling, and interdisciplinary communication.
• Attend and participate in interdisciplinary team (IDT) meetings every two weeks.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in performance improvement programs, utilization review meetings, and provide orientation and mentorship to new employees.
• Cross-train to learn new skills and assist with agency growth initiatives.
• Float to different counties as needed.
• Work 5 days per week, 8:00 AM to 4:00 PM shift.
• Required to be on call one weekend per month.

Required Qualifications

• Valid Registered Nurse (RN) license in New Jersey.
• Minimum 2 years of hospice nursing experience, including experience within the last year.
• Experience in a home health setting.
• Basic Life Support (BLS) certification.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing.

Work Schedule

• Shift: Day (8 AM – 4 PM)
• Hours per week: 37.5
• On call: Yes, 1 weekend per month
• Float: Yes, to different counties as needed

Education

• Associate's Degree in Nursing (ASN) or Diploma with RN license in New Jersey required.

Additional Information

• Participation in monthly staff meetings and performance improvement activities expected.
• Willingness to accept additional assignments and cross-train.
