Hospice RN
Location
Atlanta, Georgia
Contract Length
13 weeks
Schedule
- One opening is Monday–Friday, 8-hour days
- One opening is Friday–Monday
- Call may be required
Role Summary
Provide direct hospice nursing care to patients and supportive care to families. The role also includes interdisciplinary care coordination, patient assessment, care planning, documentation, and supervisory oversight of aides and other team members.
Required Qualifications
- Associate of Science in Nursing / Associate Degree in Nursing
- Valid RN license in Georgia
- Hospice experience within the last year
- 1 year of hospice experience
- BLS issued by American Heart Association
- Driver's License
Key Responsibilities
- Complete initial and ongoing patient assessments
- Develop and implement the hospice plan of care with the interdisciplinary team
- Communicate changes in patient condition to the attending physician and hospice physician
- Maintain accurate clinical documentation and records
- Provide education to patients, families, and caregivers related to the plan of care
- Supervise aides and complete required supervisory visits and competency oversight
- Participate in quality improvement, staff meetings, and on-call coverage when assigned

- 5–6 patients per day
- Routine, recertification, death, on-call, and PRN visits
- 60-mile travel radius from the branch
- HCHB is strongly preferred
