Description

Hospice RN

Location

  • Hermantown, Minnesota

Contract Length

  • 13 weeks

Schedule

  • Monday–Friday, 8:00 AM–5:00 PM
  • May be on call nights and weekends

Role Summary

  • Provides direct hospice patient care and supportive care to families
  • Manages assigned patient clinical records and coordinates care with the interdisciplinary team
  • Performs patient assessments, care planning, education, supervisory visits, and follow-up to support quality hospice care

Required Qualifications

  • Associate of Science in Nursing / Associate Degree in Nursing
  • Valid RN license in Minnesota
  • Hospice experience within the last year
  • 1 year of hospice experience
  • Oncology experience
  • Long Term Care experience
  • Home Health experience
  • BLS issued by American Heart Association
  • Driver's License
