Home Health Physical Therapist
Location
West Branch, Michigan
Schedule
- Monday–Friday, 8a–5p
- No call
- 60-mile travel radius
- Estimated 6 patients/day
Contract Length
13 weeks
Required Qualifications
- Valid Physical Therapist license in Michigan
- Bachelor's or master's degree in physical therapy
- Experience in Supervisory roles
- Driver's License
Experience / Skills
- Knowledge of physical therapy principles, concepts, and methodology appropriate for terminally ill patients
- Problem-solving and decision-making ability
- Ability to manage patient care with knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention
- Excellent communication skills
- Ability to develop and maintain rapport with patients, families, and staff
- One year or more of physical therapy work experience in a multidisciplinary setting is preferred
Key Responsibilities
- Develop and implement patient physical therapy plans of care
- Complete assessments, evaluations, and OASIS assessments
- Document care and maintain patient medical records
- Educate patients, families, and team members
- Participate in interdisciplinary team meetings and quality improvement activities
- Supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides as needed
