IMPORTANT - READ ALL NOTES: PLEASE MAKE SURE THAT THE ADDRESS ON YOUR TRAVLER’S DRIVER’S LICENSE IS MORE THAN 50 MILES FROM THE FACILITY ADDRESS BEFORE SUBMITTING. The facility will not allow us to base traveler’s address on the perm tax form or any other docs, they only allow the DL address (this is why they require us to upload pics of the *DUE TO THE EXTREME NUMBER OF DECLINED OFFERS, WELLSTAR IS ONLY ALLOWING SUBMISSION TO ONE LOCATION AT A TIME MOVING FORWARD* Covid Vaccine required for WelStar Kennestone location only *8 or 13 week contracts, please let me know in submit what traveler wants* For Submit, MUST HAVE Copies of: Pics of required certs for position Resume/P3 License (nursys is accepted for RNs) Pic of drivers license Pic of social security card Pic of Covid19 Vaccine ALSO, FOR WELLSTAR SUBMITS, please discuss/answer the below questions with traveler and send responses with your submit: 1. All offers are auto offers, is your traveler good to accept an auto offer only? 2. Has the traveler sent any and all RTO – any additional will not be approved after we submit them in the WellStar system 3. Is the traveler aware that they must float amongst other WellStar campuses in the area (floating guide available – please let me know if you need it)? If this is not a float pool position, floating will only be required if the census is low at their home facility, then the traveler would be sent to another campus (within 20-30 min) instead of cancelling them all together. I the traveler rejects the float offer, they will not receive their guaranteed hours and could be cancelled. 4. Does the traveler understand that they can’t request/require block schedules? WellStar does not approve blocking, but the facility will try to coordinate their schedule as best as they can after the traveler begins the assignment.
