Description
VIBRA TRAVELS is seeking a CORPORATE TRAVEL is seeking a Hospital Case Manager/LCSW to join our expanding network of healthcare professionals. We’re looking for dependable, detail-focused travelers with experience in hospital settings. If you're ready to assist while traveling the country, this is your opportunity!
Facility Type: (Select 1) Short Term Acute Care, Long Term Acute Care, Inpatient Rehab
Unlike working for agency after agency, with our travel program, you get to work for a company:
• Work with 1 recruiter from start to finish and assignment to assignment
• Local and Non-local positions available
• Competitive hourly rate in addition to tax-free stipend (if applicable)
• No pay cuts mid-contract
• Referral bonus (up to $1,000 per referral!)
• License reimbursement if traveling to new state where new license is required
• Partner facilities from various health systems across the country
Minimum Qualifications:
· Current Licensed Clinical Social Worker (LCSW) license in the state of practice
· Minimum of 2 years of clinical social work experience, preferably in a hospital or healthcare setting
· Strong knowledge of medical social work, discharge planning, and community resources
· Current, valid, and active driver’s license required.
· Ability to travel using publicly available air and ground transportation
· Recent acute care experience for acute care and LTAC placements
Other Skills and Competencies:
· Excellent interpersonal, communication, and collaboration skills
· Ability to manage complex cases in a fast-paced, high-acuity environment
· Strong clinical judgment and problem-solving abilities
· Knowledge of healthcare systems, insurance, and regulatory requirements
· Cultural competence and commitment to patient-centered care
· Proficiency with electronic medical records and documentation standards
Key Responsibilities
· Conduct comprehensive psychosocial assessments to identify patient needs, risks, barriers to care, and available supports
· Collaborate with physicians, nurses, case managers, and other interdisciplinary team members to develop and implement individualized care and discharge plans
· Provide crisis intervention, counseling, and emotional support to patients and families during hospitalization
· Assess and address social determinants of health including housing, financial resources, transportation, food insecurity, and safety concerns
· Coordinate post-acute services such as home health, skilled nursing facilities, rehabilitation, hospice, behavioral health services, and community-based resources
· Advocate for patients and families to ensure access to appropriate services and continuity of care
· Assist with complex cases involving abuse, neglect, substance use, mental health concerns, guardianship, and end-of-life planning
· Ensure compliance with hospital policies, regulatory requirements, and accreditation standards
Document assessments, interventions, and care plans accurately and timely in the electronic medical record (EMR)
· Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives
Certificates, Licensures, Registrations
· ACM or CCM may be required by the client
· Current, valid, and active license to practice as a LCSW in the state of employment required.
· Current BLS certification from a Vibra Travles-approved vendor required, AHA preferred and required by some clients.
