Job description:

Title: Physical Therapist
Facility: Hackensack Meridian Health
Location: 1340 Campus Parkway, Wall Township, NJ 07753
Duration: 13 weeks
Shift: Day shift
Target start date: ASAP

Local rates:
40 hours per week (Within 50 miles from the facility)
$88/hour (40 Regular hours)
$3,520 gross per week

Required Skills, Experience, Education & Certifications:

Required Education: Bachelor’s Degree in Physical Therapy (PT)

Required Years of Experience: minimum 1 year 

Required Licenses & Certifications: Certified Home Care experience (Part A) with OASIS knowledge and current usage. EPIC preferred. Registered Physical Therapist. BLS must be AHA.
 

Note: We need a PT who already has certified (Medicare) home care experience and knows how to complete an OASIS assessment. Ideally, we also want someone who knows how to use EPIC documentation.
