Location: Jersey City, NJ
Schedule: Full-time (split between two buildings)
Travel: Works at primary facility + one additional building 15 minutes away

About the Role

We are seeking a consistent and experienced Speech-Language Pathologist (SLP) to provide high-quality therapy services within a sub-acute rehab setting. This position involves working across two nearby nursing home buildings, offering support to diverse adult and geriatric patient populations.

Key Responsibilities

  • Evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders

  • Develop individualized treatment plans based on patient needs and goals

  • Provide therapy services within sub-acute rehab and long-term care environments

  • Collaborate with interdisciplinary teams, including PT, OT, nursing, and case management

  • Complete documentation according to facility and regulatory requirements

  • Support PDPM-aligned therapy planning and reimbursement accuracy

Required Skills & Experience

  • Sub-acute rehab experience preferred

  • Strong understanding of PDPM reimbursement or willingness to learn

  • Excellent communication, collaboration, and patient-education skills

Required Licenses & Certifications

  • Active SLP license (state of New Jersey)

  • CCC-SLP preferred; CFY may be considered depending on facility requirements

Work Environment

  • Nursing home / sub-acute rehab setting

  • Consistent caseload

  • Work split between two buildings located 15 minutes apart

  • Supportive interdisciplinary team

for the following shift(s): Days
