Location: Watertown, NY
Start Date: June 29, 2026 Shift: 8-Hour Day Shifts Contract Duration: 13 weeks
Radius Rule: Candidates must reside more than 100 miles from the facility to qualify.

Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a Long-Term Care and Sub-Acute Rehabilitation team. The ideal candidate will have experience caring for geriatric and medically complex patients and be comfortable working in a fast-paced post-acute care environment.

Responsibilities
✔️Provide direct nursing care to residents and rehabilitation patients.
✔️Administer medications and treatments in accordance with physician orders and facility policies.
✔️Monitor patient conditions and promptly report changes to the healthcare team.
✔️Complete accurate and timely documentation using PointClickCare (eMAR).
✔️Collaborate with physicians, RNs, therapists, and interdisciplinary team members to ensure quality patient care.
✔️Support patients with chronic illnesses, diabetes, dementia, and other long-term health conditions.
✔️Educate patients and families regarding treatment plans and care needs.
✔️Maintain compliance with facility, state, and federal regulations.
Requirements
✔️Active New York LPN License.
✔️Current American Heart Association (AHA) BLS Certification.
✔️Minimum 1–3 years of LPN experience required.
✔️Long-Term Care (LTC) and/or Skilled Nursing Facility (SNF) experience accepted.
✔️Strong clinical, communication, and documentation skills.
✔️Experience with PointClickCare (eMAR) preferred.
