Position Overview
We are seeking an experienced Registered Nurse (RN) Care Manager to support care coordination across a hospital setting. The ideal candidate will have strong clinical experience along with expertise in care coordination, utilization review, and discharge planning. This role focuses on ensuring smooth patient transitions, optimizing resource utilization, and supporting safe and effective discharges.
Key Responsibilities
• Coordinate patient care and discharge planning
• Perform utilization review to ensure appropriate level of care and services
• Collaborate with providers, nurses, and interdisciplinary teams for care planning
• Facilitate transitions to home care or other healthcare services
• Assess patient needs and develop effective care plans
• Maintain accurate documentation and ensure compliance with policies
• Support patients and families with ongoing care coordination
Required Qualifications
• Active RN License in the State of New York (Required)
• Minimum 5 years of clinical experience as a Registered Nurse
• Experience with care coordination, utilization review, and discharge planning
• Bachelor’s degree in Nursing (BSN) required
• Strong computer literacy skills
• Leadership experience preferred
• Experience with home care and discharge processes preferred
