Shift: : Mon- Fri 3:00 PM – 11:30 PM
Location: Newport Beach, CA, 92660
Position Overview
Seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to provide hospice care services under the direction of a Registered Nurse or Physician. The LVN will assist in assessing, planning, implementing, and evaluating patient care while supporting terminally ill patients and their families. The ideal candidate will have hospice experience, strong clinical documentation skills, and a thorough understanding of hospice philosophy and end-of-life care.
Key Responsibilities
Provide direct patient care within LVN scope of practice.
Assist with patient assessments, care planning, implementation, and evaluation.
Administer prescribed treatments and interventions.
Monitor patient conditions and report changes to supervising RN/Physician.
Implement and document discharge planning.
Maintain accurate patient records and EMR documentation.
Collect and handle specimens as needed.
Ensure HIPAA compliance and patient safety standards.
Collaborate with interdisciplinary healthcare teams.
Assist with equipment maintenance and quality measures.
Support patients and families through end-of-life care needs.
Required Qualifications
Current California LVN License.
Current AHA BLS Certification.
Minimum 1 year of healthcare experience.
Understanding of hospice philosophy and terminal illness care.
Valid driver's license and insured vehicle.
Experience with EMR documentation.
